Liability Form

• Hunter and their guests need to sign the liability form below. Non-guided hunters can bring four (4) guests.

  • Each hunter and their guest must sign the two release forms: the Wild Country Inc. and Deseret release forms. When signing these forms, you will enter the signer’s name twice (once on the Wild Country release form and once on the Deseret release form).

  • Each hunter and guest will have their own submission.

  • If the hunter or guest is a youth, the parent or guardian must sign the “Ward section” on the youth’s release form.

    • If the signer is not a youth, type N/A in the “ward section” on both forms.

  • You are welcome to email or text this link to your guest so they can complete the liability waiver.

  • To sign the form, click the green “start signing” button below.

  • When finished signing the form, click “sign & complete” and “accept & send.”
